The Fountainview Center for Alzheimer’s Disease is the first community in the State of Georgia developed exclusively for individuals with Alzheimer’s Disease and related disorders. A specialized center that addresses the progressive nature of Alzheimer’s Disease and related dementias with a unique continuum of care that provides services for mild cognitive difficulties to those with complex cognitive and medical issues, to the late stages of dementia. Our services include: Nursing services provided 24 hours a day, 7 days a week, by professional licensed Registered Nurses, Licensed Practical Nurses and Certified Nursing Assistants all trained in dementia care. Short-Stay is for those residents with a dementia diagnosis needing qualified therapy services for a shorter period of time, post hospitalization. Respite Care is provided for those families needing to take care of themselves, whether it’s a short vacation, the need to spend some time with others, respite from your care giving duties or medical necessities. You will find comfort in leaving your loved one with our caregivers experienced in dementia care. Hospice Services are available in-house. There is no need to discharge to a hospice facility, as a part of Fountainivew’s continuum of care; we encourage and support the hospice team, as they enhance our medical services when families are facing a difficult time. Social Services are a necessary and integral component of comprehensive resident care, recognizing the interrelationship of physical, psychological and social functioning, the social worker is an advocate for the resident and their families to assist in meeting the total needs of our residents. We offer the services of a full-time Social Services Director to ensure our residents in maximizing their living experience. Additional Medical Services
